Care Measured brief PHQ4 screening for anxiety and depression

Are you interested in a free self-assessment that screens for anxiety and depression? Look no further!

One of our partner therapists, Dr. Orlando Wright, LCSW-C, Founder of Halcyon Clinical Services, developed an app called Care Measured.

This 7-question survey will help you determine where you are in terms of needing mental health support. In the first portion, the PHQ4, here is the scale:

  • Normal is 0-2

  • Mild is 3-5

  • Moderate is 6-8

  • Severe is 9-12
